Well, if you have the full Visual Studio, I recall the old 16-bit edition had a pretty decent editor, which supported multiple icons in a single file, etc.
IconMaster is an excellent program to convert bitmaps (BMP) to icons. You can even provide multiple bitmaps to create multi-format icons.
IconShop is an excellent program for extracting and viewing icons and icon resources in files.
I've yet to find a really good freeware icon editor, but Junior Icon Editor is a very user-friendly tool for editing and managing many different kinds of icon format. However, it's limited to a 15-day trial (and costs $20.)
